Bringing over 12 years of leadership experience to Smoky Mountain Nursing and Rehab, Shayna has led a purposeful journey into Long Term Care, motivated by the experience of witnessing her Great Grandmother’s decline in a nursing home. As the Administrator, Shayna assumes a crucial role in leading our facility, acting as a cornerstone of our community. With a comprehensive background in operations management, regulatory compliance, and specialized training in caring for individuals with cognitive decline/dementia. Shayna is committed to elevating residents’ lives, enhancing daily experiences, and guiding our team towards the future with thoughtful and compassionate approaches. She is dedicated to cultivating a dynamic and thriving community at Smoky Mountain.

With a distinguished nursing career spanning 43 years, Tracie Rich has dedicated 28 years specifically to the realm of long-term care and rehabilitation. As the Director of Nursing at Smoky Mountain Nursing and Rehab, Tracie is driven by a deep and personal commitment to delivering the highest standard of care to every resident who walks through our doors, regardless of the duration of their stay.

Tracie’s wealth of experience and passion for nursing is evident in her hands-on approach and dedication to ensuring the well-being of each individual under her care. Her expertise extends to all aspects of the nursing department, medications, and the comprehensive care provided within our facility.

As the Referral Management Coordinator with Smoky Mountain Nursing and Rehab, my purpose is to bring peace and kindness to families in their most vulnerable times of placing a loved one in someone else’s care. I want for residents and families to feel loved, heard, and at ease when placing their loved one at Smoky Mountain.”

As the Referral Management Coordinator at Smoky Mountain Health & Rehab, Logan Hayworth brings a long-term background in healthcare, with 16 years as a CNA and the last 11 years specializing in Admission and Referral Management. Logan possesses an extensive knowledge base on Short Term, Assisted Living, and Long Term Care. Dedicated to effective communication with administrative staff, Logan serves as a single point of contact for the admissions process, alleviating the administrative burden for new residents and their families, and ensuring exceptional care, whether for short-term or long-term needs.
